Failure to warn of incompatibility with some phones – I got H or SOS only coverage through my stay overseas in NYC. There was no warning on the package that some phones, including mine, are incompatible with the Ultra service. Ultra's Customer service wouldn't even acknowledge the problem much less provide a refund, only said that it was made clear in the activation process which only occurs after you purchase the product. See top of screenshot.
