I have worn it on both.. When my daughter was only a few months old I wore it on my waist. Now that she's one I seem to be wearing her lower. For my 3 year old I definitely wear it on my hips.. I suppose it depends on the size of the child and what's comforable for us.. Hope I could offer some help..

What Lucy said. But if you are interested in trying one I would highly recommend to go to a babywearing meet. You can try different carriers and have assistance with experienced wearers
